Care Management Services
A care manager is a professional who provides advice, guidance, coordination, and resources to individuals who need some level of care. The care manager will work with families and support systems to establish tailored care solutions.
The level of care a person with a disability requires falls across a broad spectrum. Sometimes the person can reside independently in their home with limited drop-in supports. Other times the medical complexity may require 24 hour care in a skilled nursing facility. Our care managers work with a vast network of medical, financial, legal and vocational parties to make sure the appropriate level of care is established and sustained.
Midwest Care Management Services is especially able to assist in complex and difficult cases in which the individual with an impairment and/or disability is resisting care. We aim to establish relationships within the support system to develop the best plan of care.
