Stories About Our Work

My client mentioned to me that she used to enjoy crocheting so for Christmas I wrapped up a hook and ball of yarn. Based on past experiences with her, I was fully under the assumption that she would not actually use the items but enjoy the gesture.
Today I went to see her and she presented a beautiful crocheted scarf! She told me she didn’t remember how to crochet but just made it up as she went along (honestly so impressive).
Needless to say, this served as a beautiful reminder to never underestimate our clients as they can still do some pretty amazing things.

Finding Comfort in Care: Patty's Journey

Thank you for being Patty’s care manager all these years. Because of your knowledge, friendliness, and always keeping us informed, we have always known Patty was receiving the best possible care. We felt relieved you were able to help move her out of her home. As a family we were extremely frightened for her safety living alone. We knew it was your concern, too. Thank you for your kindness and understanding. Thank you again for everything. -- Nancy, Ian, Joe and Robin

Finding Purpose in Our Work

As a Care Manager, I’ve had the privilege of supporting over 30 clients, aged 21 to 95, in navigating challenges like dementia, developmental disabilities, and mental health impairments.

For many of my clients who lack decisional capacity to make medical or financial decisions for themselves, I step in as their court-appointed Guardian, advocating for their best interests, creating and executing complex care plans, and ensuring they enjoy the highest quality of life possible. My work ranges from appearing in court, drafting court reports, conducting assessments, attending client medical appointments, managing medically complex clients, and crisis intervention. I also interface between health care providers, physicians, attorneys, trust officers, and family members, including placing and supervising agency caregivers.

As a Care Manager, I have developed a skill set which includes a deep understanding of the challenges facing older adults and individuals with disabilities, including the difficult topics of aging in place and end-of-life decision making. I enjoy the meaningful relationships I build with my clients and sharing laughter, tears, and moments of understanding with my clients is a part of my role that I cherish.

Finding Connections Through Humor

As a Care Manager, I really enjoy making connections with my clients, especially guys like Pete. He is a gentle guy who likes cars, golf, and chocolate chip cookies. Pete was a lifelong Navy man and an active member in his church. His pastor visited him every week at Belmont Village and later at Generations. He is very well regarded by his church community as someone who would always step up and help out. I visited him weekly for years, and at each visit, he and I had a little schtick where I would say “How are you feeling, Pete?” And he would reply “With my hands.” And we would both smile and start catching up on the week. I’m really honored that we got to help Pete in his later years and that I got to know such a good guy.
