Midwest Care Management Services Inc. is your partner in ensuring that clients who are elderly or have physical and cognitive impairments are living their healthiest, happiest and most independent lives. Since 2010, we

have worked with families, attorneys, financial institutions, long term care facilities, medical providers, and others to coordinate care for our clients. 

Care Management

People with mental and physical impairments may have many challenges that go beyond the important assistance of their current support systems.  Our experienced and compassionate Care Managers give you one place to turn for all of your evolving needs, by:

  • Getting to know you, earning your trust, and understanding your unique strengths and challenges
  • Creating a plan to address your health, safety, daily care giving and social needs
  • Serving as a liaison between invested parties
  • Finding the best nursing home, assisted living, or supportive arrangement
  • Advocating for the best care and treatment at hospitals and facilities
  • Coordinating end-of-life planning and logistics

 

Guardianship

Sometimes our loved ones have disabilities or challenges that prevent them from making responsible personal or financial decisions for themselves or keeping up with their personal affairs. You may live far away or not be able to help as much as you’d like due to family, work, or other obligations. 

Midwest can act as your loved one’s guardian, stepping in their shoes and making those decisions for them, and helping with ongoing household or financial upkeep. We realize that this is a very serious responsibility. Our experienced and reliable team works closely with you to ensure we are incorporating your loved one’s values and best interests in every decision. 
  • Securing and managing assets, income and investments, as well as paying bills.
  • Screening and hiring caregivers.
  • Making sure you are in the best possible living situation and being treated well.
  • Coordinating all aspects of healthcare (i.e. assisting with healthcare decisions, going to doctor’s appointments, consenting to procedures.

Power Of Attorney

The experts at Midwest can help you plan ahead by arranging to be your financial or healthcare power of attorney. In that vital role, we will get to know your desires, priorities and values, and keep your wishes at the center of every decision.

As Financial Power of Attorney, we will:

 

  • Assist with investments and finance (i.e. make bank transactions, pay bills, file tax returns.)
  • Manage residence and property (i.e. review and process mail, arrange care for pets.)
  • Help with selection and enrollment in insurance and benefits.

 

As Healthcare Power of Attorney we will:

 

  • Assist in healthcare decisions and consent to medical treatments.
  • Work with clients to develop plans that enable them to stay at home or in senior living and advocate for them in   every setting.
  • Continue to keep important people in your life informed.

 

Educational Advocacy

Midwest Children’s Advocacy Group helps children, adolescents and young adults with disabilities live their healthiest, happiest and most independent lives. We do this by advocating on their behalf at school, helping coordinate every aspect of their medical care and daily needs, and working with you to envision and plan for their future. 
  • Helping all parties work together to design, implement, and monitor an IEP that addresses the student’s   individual strengths and areas of need.
  • Reviewing documentation for school programming and placement.
  • Attending IEPs and school meetings.
  • Consulting with legal teams, guardians, parents, and families
  • Supplementing school services with individualized home and private supports.
  • Providing ongoing oversight for all services.
