My client mentioned to me that she used to enjoy crocheting so for Christmas I wrapped up a hook and ball of yarn. Based on past experiences with her, I was fully under the assumption that she would not actually use the items but enjoy the gesture.
Today I went to see her and she presented a beautiful crocheted scarf! She told me she didn’t remember how to crochet but just made it up as she went along (honestly so impressive).
Needless to say, this served as a beautiful reminder to never underestimate our clients as they can still do some pretty amazing things.

Fostering Connections and Providing Support

  • As a Care Manager, I’ve had the privilege of supporting over 30 clients, aged 21 to 95, in navigating challenges like dementia, developmental disabilities, and mental health impairments. For many of my clients who lack decisional capacity to make medical or financial decisions for themselves, I step in as their court-appointed Guardian, advocating for their best interests, creating and executing complex care plans, and ensuring they enjoy the highest quality of life possible. My work ranges from appearing in court, drafting court reports, conducting assessments, attending client medical appointments, managing medically complex clients, and crisis intervention. I also interface between health care providers, physicians, attorneys, trust officers, and family members, including placing and supervising agency caregivers.
  • As a Care Manager, I have developed a skill set which includes a deep understanding of the challenges facing older adults and individuals with disabilities, including the difficult topics of aging in place and end-of-life decision making. I enjoy the meaningful relationships I build with my clients and sharing laughter, tears, and moments of understanding with my clients is a part of my role that I cherish.
  • As a Care Manager, I really enjoy making connections with my clients, especially guys like Pete. He is a gentle guy who likes cars, golf, and chocolate chip cookies. Pete was a lifelong Navy man and an active member in his church. His pastor visited him every week at Belmont Village and later at Generations. He is very well regarded by his church community as someone who would always step up and help out. I visited him weekly for years, and at each visit, he and I had a little schtick where I would say “How are you feeling, Pete?” And he would reply “With my hands.” And we would both smile and start catching up on the week. I’m really honored that we got to help Pete in his later years and that I got to know such a good guy.

Testimonials from Families and Professionals

From Clients and Families:

  • I am writing to express my sincere gratitude for the outstanding care that our Care Manager has been providing to my father. His dedication, compassion, and professionalism have made a significant difference in my father's quality of life. His commitment to excellence in his duties is truly commendable. He consistently goes above and beyond to ensure my father's needs are met. His patience, kindness, and ability to build a rapport with my father are invaluable. I believe that his exceptional care is a testament to his selfless service to others.
  • Thank you for being Patty’s care manager all these years. Because of your knowledge, friendliness, and always keeping us informed, we have always known Patty was receiving the best possible care. We felt relieved you were able to help move her out of her home. As a family we were extremely frightened for her safety living alone. We knew it was your concern, too. Thank you for your kindness and understanding. Thank you again for everything.
  • When you are in the middle of it there's so much stress. With Midwest Care, I didn't have to look around or try to guess the best  way to take care of my uncle. They knew.

From Attorneys:

  • Our goals are all the same – number one is the clients’ well-being. Midwest tries to make that happen regardless of the circumstances.
  • Ben Topp and his team are top notch. Top of the line professionals with very caring hearts. I’m so happy when I’m able to work with them!

Having Midwest Care Management's assistance is truly like having a family member at your side during difficult times. I highly recommend them!
  • I’ve worked with Midwest Care Management Services in Chicago for many years, and I can honestly say their team is something special. Their case managers truly care — not just about checking boxes, but about really looking out for the people they serve. You can feel that their clients’ well-being is always the priority. They’re led by someone who genuinely wants every client to have a high-quality, meaningful, and long-lasting quality of life, and that leadership really shows in the way the whole team operates. Over the years, I’ve seen firsthand how thoughtful, compassionate, and consistent they are with every single person they work with.
One of the things I appreciate most is how well they collaborate with families and other professionals. They communicate openly, work as a team, and make sure their clients get the best care and oversight possible. It’s clear they’re in this work for the right reasons.
